Complaints

If you believe your privacy rights have been violated, you may file a complaint with us by contacting info@vineandbranch.health, or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Effective date

This notice is effective April 2026. We reserve the right to change this notice and to make the revised notice effective for medical information we already have about you as well as any information we receive in the future. The current notice will always be posted on our website with its effective date.
